Kidney Failure, Diabetes, Hypertension, Potassium 6.2, Creatinine 6.4

I am 57 years old patient with Kidney Failure, and I have diabetes and high blood pressure. My potassium level went up to 6.20 and creatinine is 6.40. Please advice me. I need your help. Thanks!
