Diuretic Used in Renal Failure

diuretics in renal failureDiuretic is commonly used in disease characterized by fluid retention like heart failure, kidney failure and liver kidney. For kidney failure patients, aside from reducing excessive fluid in the body, diuretics also help to control blood pressure symptom. Therefore, diuretic plays an important role in the treatment for kidney failure.

In clinic, there are different kind of diuretics with different treatment effects in kidney failure, but almost all of them can help to alleviate swelling and high blood pressure symptoms. In kidney failure, with different illness condition, patients need to take different kind of diuretics:

1. Thiazide: common Thiazide includes chlorothiazide and chlorthalidone. They work by stopping distal convoluted tubule, a portion of kidney, from reabsorbing sodium. High sodium level will lead to fluid retention and high blood pressure, so when sodium level is controlled, swelling and high blood pressure will be alleviated effectively.

2. Loop diuretics: The major medicine in loop diuretics is furosemide which always cause lots of side effects like low sodium, low potassium, discomforts of intestines and stomach, low blood pressure and ototoxicity. Therefore, it is better for kidney failure patients not to take furosemide until it is really necessary.

3. Potassium-sparing diuretics: Most of the diuretics can cause potassium loss and only few medicines have function to preserve potassium. Common potassium-sparing diuretic is spironolactone. As potassium-sparing diuretics can cause high potassium level, patients need to follow low-potassium diet when take this medicine.

4. Osmotic diuretics: osmotic diuretics include Mannito and Urea which control blood pressure and reduce fluid retention by bringing fluids out of patient’s body with crystal.

Diuretics is the necessary medicines during the treatment for kidney failure. However, because of its side effects, patients need to get their doctor’s permission and have a general understanding about this medicine before taking it.
